Caribbean Deploys Advanced Genomic Sequencing to Combat Drug-Resistant Superbugs

The Caribbean Public Health Agency has launched a whole-genome sequencing service at its medical microbiology laboratory in Port of Spain. The move gives the region a substantially improved capacity to identify, track and respond to antimicrobial-resistant pathogens, a class of infections that has grown steadily harder to treat with standard antibiotics.

Antimicrobial resistance occurs when bacteria evolve to withstand the drugs designed to eliminate them. Infections that were once treated routinely can become prolonged, more expensive to manage and, in some cases, fatal. Multidrug-resistant strains have become more common in hospitals and in the wider community, and the trend has exposed a gap in diagnostic capability across parts of the Caribbean, where access to advanced molecular screening has historically been limited. Health officials in the region have pointed to this gap as a pressing concern, particularly as resistant infections do not respect national borders.

The new service represents a marked departure from older diagnostic methods. Traditional susceptibility testing exposes a bacterial sample to different antibiotics and observes which ones fail to stop its growth. Whole-genome sequencing works differently. It maps the entire genetic code of a microorganism, allowing scientists to identify the specific mutations responsible for resistance. This level of detail means investigators can trace how a resistant strain has moved between patients, hospitals or even countries, and can flag which strains pose the greatest risk during an active outbreak. Where older methods might tell a clinician that a bacterium is resistant, genomic sequencing can explain why, and where it likely came from.

The initiative has been developed with international backing. It is supported by the UK Health Security Agency and the Fleming Fund, a programme run by the UK Department of Health and Social Care that finances efforts to tackle antimicrobial resistance in low and middle-income countries. The Port of Spain laboratory upgrade forms part of a wider set of internationally funded projects aimed at strengthening laboratory networks across the Caribbean, work that has taken on added urgency following recent global outbreaks that exposed weaknesses in regional testing infrastructure.

Officials at the Caribbean Public Health Agency have described the new service as a significant step for health security in the region. Leadership at the agency has framed genomic surveillance as central to a more evidence-based approach to outbreak response, one that allows decisions to be grounded in precise data rather than partial information gathered after an outbreak has already spread. The head of the laboratory has focused on the practical gains the technology offers, noting that it provides far greater insight into the specific genes driving resistance and allows investigators to reconstruct the path of an outbreak with much greater accuracy than before.

In practical terms, the change is immediate. Member states across the Caribbean can now send bacterial samples directly to the Port of Spain facility for genomic analysis, rather than relying on more limited testing elsewhere or sending samples overseas, a process that has often delayed results. The agency has said it expects demand for the service to grow as more countries in the region become aware of its capabilities.

The longer-term ambition is to build a public health network across the Caribbean that can detect and respond to infectious disease threats before they escalate. Genomic surveillance of this kind has already reshaped how larger health systems track outbreaks, and its arrival in the Caribbean marks an attempt to close the resource gap that has left smaller nations more exposed. Whether the facility can keep pace with the volume of samples a growing regional demand may generate remains to be seen, but its launch gives Caribbean states a diagnostic tool that was, until now, largely unavailable to them.
